Minimum 3.3 Cumulative GPA
Intent to Pursue a College Education
Senior Student in Good Standing at West Monroe High School

Completed Application
Copy of your ACT Score
Two Letters of Recommendation (All recommendation letters must include students’ first and last names for verification purposes.)
Current Color Photo of Yourself (Wallet size up to 4x6; yearbook photo preferred; no photocopies please; student should be dressed appropriately for use in our newsletter with shoulders covered.)
2 Copies of Your Personal Essay (Instructions found on the application)
A good contact phone number for the student and/or parent to notify recipients of the awards
